MOMS Care Coordination Model

Source: Maternal Opiate Medical Support

Policymakers and clinicians in Ohio developed the Maternal Opiate Medical Support (MOMS) model of providing prenatal, obstetrical, and postpartum care for pregnant women with opioid use disorder. This guide includes a process to assess readiness to adopt the MOMS model and an implementation guide for clinics and practices, as well as algorithms to guide clinicians and care coordinators in delivering the MOMS model to their patients. The guide emphasizes how providers can work as a collaborative team to provide needed clinical, social, and behavioral health services to their clients.
