Guidance for clinicians on how to taper patients’ long-term opioid prescriptions to lower dosages or discontinuation from the U.S. Department of Health and Human Services. Includes a list of indications for tapering, stresses the need to work with the patient and engage in collaborative, voluntary tapering plans, and provides recommendations for how to individualize the taper rate. Also includes guidance on how to treat withdrawal, how to recognize opioid use disorder, and how to address special populations including patients who are unable or unwilling to taper their opioid dose.
