This 2018 toolkit from the California Quality Collaborative and Smart Care California provides step-by-step guidance for provider organizations to change provider behavior and culture around opioids. The toolkit is organized around 4 key strategies: prevent new opioid prescriptions by adopting safe prescribing practices; manage pain safely and work with patients to find safer pain treatment plans; treat addiction effectively by increasing access to medications to treat opioid use disorder and providing additional services; and stop deaths from overdose by adopting and promoting harm reduction strategies. The toolkit was developed by reviewing evidence-based practices cited in literature, receiving advice and feedback from experts in the field, and by speaking with representatives from California health care organizations who had implemented policies in their facilities and offices. The tool-kit includes guidance on how to make the case for change and how to develop multi-year initiatives by taking the first step. The tool-kit then lists specific changes provider organizations can take with strategies for adoption and links to resources providing additional information. It includes a case study of how El Dorado County Community Health Centers adopted a model for team-based medication assisted care for opioid use disorder as well as examples from other California initiatives. The appendices include a list of all change strategies, examples of performance measures organizations have used to track their success, and a list of all resources cited by the toolkit.
